Abstract: Demographic variables include record of residence, age and sex of all family members, marital status, education and occupation. Other variables include the general health of each member of the household, information about respondent's doctor, travel time to get to doctor, waiting time to see doctor, limitations imposed by health conditions, history of pregnancy, methods of contraception employed, hospitalization history, means of paying for hospital bill, number of check-ups in last 12 months, dental history, history of smoking and history of drinking alcohol.
