Abstract: The questionnaire was designed to provide information that would permit the assessment of all families in 11 general areas: economic status of family, employment of family members, family use of health facilities, family's housing condition and practices, family marketing and consumer practices, family cohesion and relationships, mother's functioning as a parent, psychosocial functioning of children, psychological functioning and status of female family head, and helpfulness of Collaborative Service. In addition, information was secured on the client's own assessment of change in these areas since the family's first application for public assistance. Where change was reported, questions probed for information about the respondent's view of the cause of that change.
